In the nursing process, Assessment is the first and most crucial step, involving the systematic collection of comprehensive data about a client's health status. This data collection relies on two main sources and two types of information:

• Sources of Information:

  • Primary source: This is the client themselves, providing direct information about their symptoms, feelings, and health history.
  • Secondary source: This includes anyone or anything other than the client, such as family members, medical records, other healthcare providers, or laboratory results, which offer additional context and information.

• Types of Information Collected:

  • Subjective data: This is information from the client's point of view, including their feelings, perceptions, and concerns. It cannot be directly observed or measured by others (e.g., "I feel nauseous," "My pain is 7/10").
  • Objective data: This consists of observable and measurable data obtained through standard assessment techniques, physical examination, and diagnostic tests. It is factual and verifiable (e.g., temperature 98.6°F, pulse 100 beats/minute, a visible rash).
