How do I complete a B-BBEE Exempted Micro Enterprise (EME) Affidavit?

Fill in your Full Name & Surname and your Identity Number in the first two blank fields at the top of the form.

Provide the details of the enterprise: its Enterprise Name, Trading Name (if different), Registration Number (e.g., CIPC registration number), and the full Enterprise Address.

Declare the percentage of black ownership and black women ownership in the designated blank fields. Confirm that the enterprise's annual income did not exceed R10,000,000.00 (Ten Million Rand). Based on the black ownership percentage, tick the appropriate box in the table to indicate the B-BBEE procurement recognition level:

• If 100% black owned, tick "Level One (135% B-BBEE procurement recognition)".
• If More than 51% black owned, tick "Level Two (125% B-BBEE procurement recognition)".
• If Less than 51% black owned, tick "Level Four (100% B-BBEE procurement recognition)".

Confirm if the entity is an empowering supplier in terms of the B-BBEE Codes of Good Practice. Read and understand the contents of the affidavit, acknowledging its binding nature.

Sign the document as the Deponent Signature and fill in the Date. The affidavit must then be certified by a Commissioner of Oaths, who will sign and stamp the document, making it valid for 12 months from that date.
