You must fill in your personal details, the enterprise's name, registration number, and address. Declare the percentage of black ownership and black women ownership, and confirm the annual income did not exceed R10 million. Based on the black ownership, tick the appropriate B-BBEE procurement recognition level (Level One, Two, or Four) in the table.
