Trauma management for an EMT involves a systematic approach to assess and treat patients with injuries. The primary goal is to identify and manage life-threatening conditions quickly.
Long Question Approach:
- Scene Size-up: Ensure the scene is safe for you and your partner. Identify the mechanism of injury (MOI) and the number of patients. Don appropriate Personal Protective Equipment (PPE).
- Primary Survey: This is a rapid assessment to identify and treat immediate life threats.
- Airway: Open and maintain a patent airway. Use jaw-thrust maneuver if spinal injury is suspected.
- Breathing: Assess for adequate breathing. Administer oxygen if needed.
- Circulation: Check for pulse, skin condition, and significant bleeding. Control external hemorrhage.
- Disability: Assess neurological status using the AVPU scale (Alert, Verbal, Pain, Unresponsive).
- Exposure/Environment: Expose the patient to assess for further injuries while preventing hypothermia.
- Resuscitation: Intervene immediately to correct life-threatening conditions identified in the primary survey (e.g., administer oxygen, control bleeding, perform CPR).
- Secondary Survey: A more detailed head-to-toe examination performed if the patient is stable. This includes taking vital signs, obtaining a SAMPLE history (Signs/Symptoms, Allergies, Medications, Past medical history, Last oral intake, Events leading up to injury), and a physical exam.
- Reassessment: Continuously monitor the patient's condition and reassess vital signs and interventions.
- Transport: Determine the need for rapid transport based on the patient's condition and MOI.
Short Question Approach (Focus on Key Elements):
For a shorter question, focus on the most critical aspects:
- Identify Life Threats: What are the immediate dangers to life (e.g., airway obstruction, severe bleeding, tension pneumothorax)?
- Prioritize Interventions: What is the most critical intervention needed right now? (e.g., stop bleeding, open airway).
- Rapid Assessment: Quickly assess ABCs (Airway, Breathing, Circulation).
